Medication Administration

If your child requires any medication to be administered at school, this form must be submitted to the health suite. All prescription and non-prescription medication to be given in school must be ordered by a person authorized to prescribe medication. The first dose of medication should be given at home. Medication must be brought to school’s Health Suite in its original container with your child’s name on it.
